Person Centred Care

This course has been structured to give learners the opportunity to explore the key concepts of person-centred care and how they apply these concepts in their practice, within their teams and organisation.

The session contains examples of good and poor person-centred care in various health and social care settings and encourages learners to identify areas of their own practice which require additional development.

This session makes use of reflective learning techniques and encourages learners to use the reflective note-making facility to build on their understanding of this fundamental method of communication and care, and their practice.

• Describe how person-centred care can be put into practice in your role
• Differentiate between examples of good and poor person-centred practice and reflect on your own practice, the practice of your colleagues and your organisation
• Identify ways that the person’s immediate environment may influence their experience of receiving person-centred care
• Describe how person-centred care is an ongoing process and continuous discussion with the individual is required to support them effectively
• List appropriate ways staff can report concerns
