The introduction of the National Learning from Deaths framework has led to a requirement for training to support organisations in a variety of areas, including: their mortality governance; how they launch and handle investigations; maintaining a robust quality improvement strategy; embedding safety culture into their organisations; and understanding the impact of human factors on staff performance and behaviour. This learning provides uniformity and relates directly to the good working approaches included in the guide produced by the CQC. This course has been developed in partnership with Health Education England.


Course Aim

The overall aim of the elearning is to encourage a learning and quality improvement culture within the health and care sectors. It helps organisations remove barriers and enable a change in culture whereby all learning opportunities are taken.This in turn will result in improvements in care.

  • Module 5: Actions and Improvements
  • Module 6: Trust Boards and the role of Non-executive Directors
What will learners get out of it?

Having completed the learning, participants will be better placed to bring about improvements in care by:

  • Improving their communication when dealing with death
  • Encouraging learning from families, and examining how this can be carried out and supported
  • Encouraging learning from mistakes
  • Developing a practical understanding of reviews, inspections, investigations, and reports
  • Assisting the work of networks in sharing knowledge and learning

Board level staff will gain an understanding of roles, governance, goal setting, promotion of an open culture, and analysing data.

Who is the course for?

Anyone working in a health or care setting will benefit from this course, and everyone is strongly advised to study it.

The course supports the ‘ward-to-board’ responsibility of learning from deaths and serious incidents and includes learning which is relevant to all roles.


The eLearning includes a series of questions based on the content covered by the modules. This will enable participants to test their, knowledge retention and understanding.

When you have successfully completed all the modules of this course, you will be able to download a certificate of completion.
