Investigating Adverse Incidents In Healthcare

Delivering healthcare whilst ensuring patient safety is always a challenging environment. Unfortunately things occasionally do go wrong. When this does happen it is essential for healthcare providers to investigate thoroughly and, in keeping with the duty of candour, involve patients, their families and others fully involved in the investigation. The emphasis will always be on finding out why an incident took place and seeking to ensure that it does not happen again.

The Sancus Investigating Adverse Incidents In Healthcare training is an interactive two day course which will equip those charged with carrying out investigations with the knowledge and understanding too ensure that this important role is carried out successfully.

The training can either be purchased ‘in house' by organisations or individuals places on public courses can be booked.

SFJ Awards have accredited this course. The accreditation element is optional and involves preparation of an assessed piece of work following attendance on the training. Completion of the course equates to 14 hours of continuous professional development.

This course will demonstrate how an investigation should be structured and carried out. Delegates will learn the principles of planning an investigation, recovering evidence and interviewing witnesses. Incorporated in the learning is adopting a Root Cause Analysis (RCA) approach to identify the true cause of the incident.

The qualification covers the following topics:

  • Planning an Investigation
  • The Principles of Investigative Interviewing
  • Root Cause Analysis and Human Factors
  • Using the NHS Serious Incidents Framework
  • Making SMART Recommendations

Accreditation is achieved by completing a post-course on line assessment. The course can be provided without the accreditation element if required.

The course delivery involves delegates working through a scenario based situation to enable them to exercise their decision making skills. This scenario will be tailored meet typical situations they may encounter. The clients own policies and procedures will be assimilated into the delivery to ensure relevance.
