The End of Life Care Home workbook was developed in 2018 with the purpose to increase knowledge and awareness of good practice within end of life care. Compiled by Kirklees and Calderdale End of Life Care Education Group, who lead on sharing best practice and training, it has been developed specifically for care and support staff, across nursing care, care homes or in the community. It helps staff to recognise what good end of life care looks like, in order to make a difference to care.

This workbook provides information, practical tips and resources. It has been designed to act as a resource which can be accessed as and when required or used as part of training.

Using the workbook staff can expect to:

  • Feel more empowered to support people at the end of life
  • Increase their knowledge on key topics and conditions
  • Find ways in which they can improve care
  • Access further resources and learning materials

Using the Workbook The workbook can be completed at your own pace; reading through each section and completing the tasks and questions which are are designed to make you consider your role and what you may do well at the moment, as well as what you might want to do differently. This includes practical tips and things you may want to discuss with your wider team. We would expect the workbook to be completed within six months.

The workbook commences with pre-assessment questions. You will return to these at the end of the workbook to see if there has been a difference in your confidence and understanding of key issues. We would recommend that your manager informs CQC that you have completed the workbook. By completing this workbook, you will be in line with existing competencies. This has the potential to develop better outcomes for people in your care by improving your knowledge and understanding, as well as considering what you might do differently.

Content - The workbook contains chapters on the following topics: what is end of life care, communication skills, advance care planning, recognising dying, supporting someone who is deteriorating, sympton management, non malignant conditions, spirituality, bereavement.

Staff at Heatherstones have completed the End of Life Care workbook, some have worked through it on an individual basis, some have completed parts of it as a group. As a team we have found the workbook is a really useful tool to use in our service, it guides and gives information on symptom management in a concise manner, it is easy to follow, it explains things clearly. Staff say they have enjoyed working through it and that it has explained things thoroughly to them. It is a tool that we will continue to use long term; it has prompted us to challenge our thinking and to question each other. The teaching surrounding the work book was very good as was the follow up session, where we were able to ask lots of questions. I feel confident that we could contact Karen (Clinical educator) in the future should we need any support.
